Grace Modina, CNP, RN, is dedicated to providing compassionate and evidence-based mental health care to patients across Minnesota. She specializes in medication management for children, adolescents, and young adults, helping them navigate challenges related to ADHD, anxiety, depression, and other mental health conditions. Grace understands that every individual has unique needs, and she works closely with her patients to develop personalized treatment plans that support their well-being and long-term stability. Grace has a strong passion for working with individuals experiencing ADHD, autism spectrum disorder, and developmental disabilities. She recognizes the complexities of these conditions and strives to create a supportive, judgment-free environment where patients and their families feel understood and empowered. Her approach integrates mindfulness techniques alongside medication management, fostering self-awareness and emotional regulation in her patients. She is also experienced in addressing childhood behavioral concerns, PTSD/trauma, and sleep disorders, ensuring a holistic approach to care. In addition to her expertise in treating a wide range of mental health conditions, Grace is dedicated to supporting populations with unique needs, including individuals in the adoption and foster care system, those within the LGBTQ+ community, and individuals with autism spectrum disorders. Her inclusive and affirming care helps patients feel safe and respected, allowing them to build confidence in their mental health journey. Outside of her practice, Grace enjoys spending time outdoors with her partner and their dog, as well as engaging in creative hobbies like crocheting, thrifting, and reading. She believes that maintaining balance in life is essential for both providers and patients and encourages self-care as a vital part of mental health.
