Rachel York, Nurse Practitioner
Rachel York helps adults regain stability, clarity, and hope through thoughtful, evidence-based psychiatric care tailored to their individual needs. Serving patients across New Hampshire, she provides medication management with a calm, collaborative approach that prioritizes both symptom relief and long-term wellness. Rachel works with adults and seniors navigating complex mental health concerns, offering care that is grounded, respectful, and responsive to each person’s life context. Rachel is especially passionate about supporting individuals who have been recently discharged from a psychiatric hospital. She understands that this transition can feel overwhelming and vulnerable, and she focuses on continuity of care, careful medication adjustments, and close follow-up to reduce the risk of relapse or rehospitalization. Her goal is to help patients rebuild confidence, establish stability, and move forward with a clear, manageable treatment plan during this critical period of recovery. Her clinical experience includes treating ADHD, mood disorders such as depression and bipolar disorder, and anxiety-related conditions. Rachel also works with individuals facing grief, trauma-related concerns, sleep disorders, and serious mental illness. She provides medication management for mental health and substance use concerns and may incorporate faith-based counseling elements when appropriate and aligned with a patient’s values. In addition to her primary areas of expertise, she offers care for a range of additional psychiatric conditions. Rachel serves adults and seniors and has experience working with first responders, military members and veterans, and LGBTQ+ individuals. She values inclusive, affirming care and strives to create a therapeutic environment where patients feel heard and respected. Outside of clinical work, Rachel enjoys traveling, going to the movies, and spending time with her dog, Sherman.
